Patients can add their signature to our forms with a stylus or their finger on a tablet or smartphone, or using their mouse pointer on a laptop or desktop. Multiple signatures can exist on the form (even on the same page), and will be embedded into the PDF you receive when the form is submitted. Electronic signatures can be set as required fields.

We’ve found the most usable (and most popular) format to be a PDF form, although plain text, two-column HTML, CSV (Excel®), XML and refilled custom templates (HTML, XML) are also available.
