Custom, Secure Patient Forms

Just How Legal Are They?

The U.S. Electronic Signatures in Global and National Commerce (ESIGN) Act in 2000 legislated that electronic signatures are legal in every state and U.S. territory where federal law applies.

Where federal law does not apply, most U.S. states have adopted the Uniform Electronic Transactions Act.

Back in the day, there were two requirements that stopped a submittable medical form in its tracks: the need for a written signature, and the need to attach documents (more on that here). We spent many years designing forms that had a line for the user to sign on, but we had to add text that said “You’ll be asked to sign a copy of this form when you arrive in our office”. That was certainly not much fun for the office staff to keep straight...

After a while, along came the “Type your name here. You affirm that this will serve as your signature...” method, and although it was surprisingly legal in most states, it just seemed kind of lame – and patients were leery that it wasn’t official, and that anyone could “forge” their name on the form (and they were right).

The answer was allowing people to actually sign their own name online. We’ll admit that the result doesn’t look all that artistic, but it works and it’s legal. Can’t ask for more than that...

Secure patient forms using Electronic Signature

We can place as many signatures as you need on your form, and can make them required before the form can be submitted. The signature will be embedded in the PDF form that you receive after the form is submitted.
